DEPARTMENT: Home Care Agency

REPORTS TO: Home Care Director

SHIFT: 8:00 am- 4:00pm Mon-Fri. May involve some work every other weekend

CLASSIFICATION: Non-Exempt

POSITION SUMMARY:

Responsible for assisting Home Care Agency clients with personal care and homemaker needs.

Essential Duties and Responsibilities:

1. Use appropriate skills and abilities to support Home Care Agency clients with personal care needs.
2. Review and follow current care plan developed for the client, and appropriately document care provided.
3. Communicate with supervisor and Home Care Agency scheduler of availability to work specific days/hours, and provide timely requests for time off or other changes in availability.
4. Attend departmental meetings and trainings, and comply with requirements for Relias Online Learning courses.
5. Inform supervisor and nurses in the appropriate level of care of any changes or concerns with client functioning, health, and overall well-being.

Education and/or Experience:

• 2-3 years’ experience in healthcare field desired.

Certificates, licenses, registrations:

• Certified Nursing Assistant certification required.
• NC Driver's licenses and auto insurance required
